Unless you are having clothes tailor made for you then the chances are any clothes you buy won't fit perfectly. Or at least most won't. But if something doesn't fit well then it won't look as good, it won't be as flattering and you likely won't wear it as much.
I am very petite so almost any item off the peg is either too long or too big or both. Especially a more tailored item, if it's a stretchy lycra type dress that's a lot easier to find one that fits well than a tailored one because the waist and hips are usually too big.
Over the years I have learnt that sometimes it isn't worth doing. Trousers with front pleats etc are near impossible to make fit without losing the shape, but the flat fronted, side zipped ones are a lot easier to do. If you have a very tailored, lined dress then that will be an expensive job, but if you just need it shortened a bit then it won't be so bad. I also wouldn't spend a lot on having a very inexpensive item tailored as chances are it won't last long enough to be a good investment.
Having said all that, once you make the item fit you perfectly then you will likely get a lot more wear out of it and use it a lot more. So in a lot of cases it is really worth doing. That inch off the hem or the sleeve can be the difference between looking ok and looking great so do keep it in mind.
So when you buy a new item, factor in the tailoring costs too, then decide if it's worth it for you.
